What should I do if I lost my passport with US visa?

 

Q: I recently lost my Jamaican passport with my US visitor's visa inside. What do I do to report it lost and what happens if I find
it later?


A: Start by contacting your local police station and obtaining a police report for the lost passport and visa. Then contact the embassy's Consular Section. You have two ways to do this (following the steps below). We will respond to your e-mail and include a recovered passport/visa form for you to complete and return. NOTE: Should you recover your visa once you have reported lost, do not try to enter the United States with it before you have notified us that you have recovered it.

. If you plan to apply for another US visa, schedule an appointment for a visa interview. Obtain a new Jamaican passport in time for your appointment, and bring your police report with you. Our appointment wait times are very short right now (about one week). You will need to pay the appointment scheduling fee of US$10 for online scheduling or US$13 for telephone scheduling. You will also need to pay the visa application fee of US$131 at any NCB branch. When you come in, we will assist you in reporting your previous passport/visa lost.

We will reply and send you a lost/stolen visa reporting form. Complete the lost/stolen visa form and attach it, a copy of your police report, and a copy of an identification card (such as a driver's licence) in a return e-mail.

If you apply for a new visa, we will interview you the same way we interview all other applicants as required by US immigration law. During the interview, we will consider how you used your previous visa. However, we will also be looking at your current situation to determine whether you are still eligible. In certain cases, individuals who previously qualified for a visitor's visa may not qualify to receive another visa.

I need medical treatment in the US

Q: I am a diabetic and need to seek medical treatment in the United States. What type of visa do I need and what do I need to bring to my visa interview?

A: An applicant seeking medical treatment receives a visitor's visa. Make sure that you write "medical treatment" as the purpose of travel on the form, and that you tell the interviewer that is why you need to travel. You can get detailed information about the types and uses of US visas at: http://travel.state.gov/visa/temp/types/types_1262.html#1.

At your interview, you will need to demonstrate that your travel and the treatment will be paid for. First, you will need to show a medical diagnosis of your condition from a Jamaican physician, a plan for the treatment, and whether the treatment is available here in Jamaica. You will also need to provide a letter from the US doctor, hospital or other medical establishment that will be providing the treatment showing that they accept you for treatment and how much it will cost. You will then need to show either that you are financially able to cover the medical treatment, that someone else will pay for the treatment, or that the treatment will be provided at no cost. The documentation you will need will include your bank statements or documentation showing financial support from friends or relatives. Among other issues that we will consider with your application are:

. How long you will need to remain in the United States for treatment; and

. Whether you have a residence outside the United States to which you will return at the end of your visit.
